As a resident of Washington, you should take the following steps to ensure your decision to become a donor is carried out:

  • Sign up online at www.livinglegacyregistry.org.
  • Indicate your wishes on your driver’s license or state ID when you apply for or renew it.

As a resident of Oregon, you should take the following steps to ensure your decision to become a donor is carried out:

  • Sign up online in the Oregon Donor Registry at www.donatelifenw.org.
  • Indicate your wishes with a “D” on your driver’s license or state ID when you apply for or renew it.

Oregon does not require family consent to carry out your wishes to be an organ, eye or tissue donor.
